First Visit & New Patient Forms

Thank you for choosing our office for your dental needs. We have created a comfortable environment where you can feel confident in the level of dental care you receive. You may print and fill out the appropriate new patient packet provided here or complete one provided for you when you arrive at the office. These forms allow us to begin your dental treatment and will inform you of our office policies. We ask that you read them in full and complete them to the best of your ability.

We believe that good dental care begins with open communication. We promise to speak candidly with you about any symptoms present that may require further dental treatment, our diagnosis, alternative treatments that may exist, and our recommendations. Working together, we can achieve a true partnership, with one common goal-keeping your smile beautiful and your teeth, gums and jaw joints healthy!

  New patients eighteen and over

Your initial visit will help us get acquainted with each other and learn about your goals and desires for your dental treatment. It will begin with a full set of intraoral photos, a full set of x-rays and a full exam performed by your dentist in order for him to examine the jaw’s structure, teeth position and teeth decay. Your dentist will then provide you with an evaluation that will outline your existing dental problems and proposed treatment. These procedures will take approximately one and a half hours to perform, so be sure to set aside enough time.

You will not recieve a cleaning on your first visit, unless otherwise specifically indicated at the time you scheduled your appointment. If you have scheduled a cleaning appointment with the hygenist on the same day as your initial visit please be advised that this will add another hour to your appointment time.

The initial visit for our younger patients lasts approximately one hour and includes a set of four x-rays, a full exam by the dentist, and a dental cleaning and flouride application by the hygenist. Parents will then be provided with an evaluation from the doctor that will outline existing dental problems and proposed treatment.
